Garden Spot Athletic Forms and Policies  

Listed below are links to download the forms necessary for participation in all Garden Spot Varsity, Junior Varsity & Junior High Athletics.

The following forms are required and need to be completed, signed and returned to coaches by the first day of practice for participation in a high school sport and middle school sport.

  • Drug, Alcohol, and Tobacco Contract
  • Garden Spot Consent Form
  • PIAA Medical History Form
  • PIAA Physical Evaluation Form
  • 2 Emergency Procedure Cards

The Physical for the upcoming school year needs to be completed after June 1st.
